Philips L12-5 linear array vascular ultrasound transducer tested on a Philips iU22 system. Produces clear grayscale imaging and strong color Doppler. Minor cosmetic wear with a small lens surface gouge noted. Fully functional at time of testing.

Condition Summary:

Probe Housing & Handle:
Good overall cosmetic condition. No cracks or structural damage observed. Minor scuffs and light edge wear consistent with normal use.

Acoustic Lens:
Small localized surface gouge present. No delamination observed. Remaining lens surface intact. May cause minor localized artifact but does not impact overall functionality.

Cable & Strain Relief:
Approximately 7 ft cable. No cuts, kinks, or exposed wiring. Strain reliefs intact and secure.

Connector (C-BORE Type):
Connector in good condition with no bent or missing pins. Locking mechanism intact and firm.

Functionality / Testing Summary:

Air Scan:
Sharp and uniform image. No significant dropout observed.

Phantom Scan:
Clean and consistent image. No dead elements or channel loss identified.

Vascular / Carotid Scan:
Right carotid visualized with clear grayscale imaging
Color Doppler strong and responsive
No signal voids or instability observed

Overall Performance:
Probe is fully functional with stable imaging and Doppler performance. No evidence of element failure.
